The goods and services shown below are those we can provide to our customers.  You may choose only those items you desire. (However, any funeral arrangements you select will include a charge for our basic services and overhead.)  If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide the funeral goods and services you selected.
 
SERVICES OF FUNERAL DIRECTOR AND STAFF
Our professional service charge includes but is not limited to consultation with family and clergy, arrangement and direction desired, preparation and filing of necessary notices, authorization and consent; and other services and attendance prior to during and following the funeral including coordinating with those providing other portions of the funeral, e.g. cemetery, crematory, and others.  Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory expenses.

This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select.  (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

EMBALMING
Except in certain cases, embalming is not required by law.  Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing.  If you do not want embalming, you usually have the right to choose an arrangement that does no require you to pay for it, such as a direct cremation or immediate burial.

USE OF FACILITIES
  A.  Use of facilities and staff for funeral ceremony/rite/service.  Our services include coordinating the funeral arrangements, supervision of funeral and staff to attend service.
  B.  Use of facilities and staff for visitation (per day).  Our services include set-up of visitation area, placement of encased remains, display of floral arrangements, supervision of and attendance during the visitation.
  C.  Use of facilities and staff services for funeral services or additional staff necessary for services in other facility.  Our services include coordinating the funeral arrangements, supervision of funeral, and staff to attend funeral ceremony.
  D. Use of facilities and staff for memorial services (without body).  Our services include coordinating the memorial service, and staff to attend the service.
  E.  Equipment and staff services for graveside services.  Our services include accompaniment of remains to cemetery, supervision of graveside, and staff to attend service.
  F.  Shelter of remains (per day).  This is a declinable charge if our facilities are not used to shelter remains.

FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME
This includes removal of remains, services of staff, embalming, receptacle for body, necessary authorizations, local transportation to airport or railhead.  It does not include the services of staff or the use of  facilities for any visitation and/or rites or ceremonies prior to forwarding body.

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME
This charge includes temporary shelter of remains, transportation of remains to cemetery  or crematory and required services of staff.  It does not include the services of staff or the use of facilities for viewing or for funeral rites/ceremonies.

IMMEDIATE BURIALS
An immediate burial (without any attendant rites or ceremonies) includes:
   —-  removal and shelter of the remains
   —-  local transportation to cemetery
   —-  necessary services of staff and authorizations
For an additional charge, these options are available:
   1.    Immediate burial with casket selected in
          addition to the price of the casket
   2.    Immediate burial with alternative
          container
   3.    Immediate burial with container
          provided by purchaser

DIRECT CREMATIONS
Our charge for a direct cremation without any attendant rites of ceremonies includes:
   —-  removal and shelter of the remains
   —-  local transportation to crematory if local
   —-  necessary services of staff and authorizations
If you want to arrange a direct cremation, you can use an alternative container.  Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without outside covering).  The container we provide are (fiberboard).

MISCELLANEOUS CHARGES

Either a family car or a pallbearer car will be furnished for funeral service any additional vehicle will be an additional charge.
